RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AND PARENTAL CONSENT AGREEMENT
I am aware that participation in the FUNdaMENTAL Basketball Skills Camp has some inherent risks and injury can occur. On rare occasions these injuries can be serious. In consideration of my child being allowed to participate in the FUNdaMENTAL Basketball Skills Camp, I, the parent/guardian, assume the risk of all injury and agree not to sue Lilac City Legends Inc, the Bobby Jack Foundation, the camp directors, coaches, assistant coaches, agents, officers, members, volunteers, team members, and employees, other participants, any sponsors, advertisers, and, if applicable, owners and lessors of premises on which the Activity takes place for any and all injuries caused by or resulting from participating in the FUNdaMENTAL Basketball Skills Camp.
PHOTO AND VIDEO RELEASE
FUNdaMENTAL Basketball Skills Camp, and the partners associated with it (Step Back Bobby Jack Foundation and the Lilac City Legends), has my permission to use my or my child's photograph and video captured to promote the camp. I understand that the images or video may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.
